Without maintained health, daily activities can be disrupted and life itself can be cut shorter than it should be.

This is not merely a proverb. A study published in the Journal of Aging revealed a startling finding: never brushing teeth at night can increase the risk of death by 20–35 percent, while not visiting a dentist within a year increases the risk of death by as much as 30–50 percent. Furthermore, oral health has also been linked to serious conditions such as heart disease, stroke, diabetes, and even dementia.

So, maintaining oral hygiene is not just about white teeth or fresh breath is about the quality and length of our lives.

One part of the body that is often underestimated yet critically vital is the teeth. Teeth are not merely chewing tools; they are connected to a network of nerves that influence overall body health. This is why, since ancient times, humans have continuously innovated to keep them clean and one of the oldest innovations still relevant today is siwak.

What Is Siwak? Getting to Know Nature’s Toothbrush

Siwak, also known in some regions as miswak, is essentially a twig from the arak tree (Salvadora persica), a type of shrub widely found across the Middle East. The tree is known for its naturally fragrant aroma, which is why in ancient times, siwak twigs were used to clean teeth directly, without toothpaste or any additional substance whatsoever.

The method of use is straightforward: the tip of the twig is peeled, then chewed until it forms fine bristles resembling a brush head. From there, it is used to scrub the teeth, gums, and tongue. Yet, as we will explore, far from ordinary.

A Tradition Ahead of Its Time

The practice of using siwak is nothing new. In Islamic history, it is narrated that this habit was practiced as far back as the time of Prophet Ismail AS. But what makes it truly remarkable is how Prophet Muhammad SAW made it part of his daily life and actively encouraged it for the entire Muslim community.

In an authentic hadith narrated by Imam Ahmad, the Prophet SAW said:

السِّوَاكَ مَطْهَرَةٌ لِلْفَمِّ مَرْضَاةٌ لِلرَّبِّ

“Siwak purifies the mouth and earns the pleasure of Allah.” (H.R. Ahmad, authentic)

The phrase “earns the pleasure of Allah” carries a profound meaning: a clean mouth is part of physical purity, which is a condition for the completeness of worship. In Islam, acts of worship require both inner and outer cleanliness simultaneously.

Another hadith paints a picture of just how deeply the Prophet valued cleanliness in every aspect of his life:

كَانَ رَسُوْلُ اللهِ يُعْجِبُهُ التَّيَمُّنُ فِيْ تَنَعُّلِهِ وَتَرَجُّلِهِ وَطُهُوْرِهِ وَفِيْ شَأْنِهِ كُلِّهِ

“The Prophet SAW always preferred to begin from the right side when putting on his sandals, combing his hair, performing purification, and in all of his important affairs.” (Muttafaqun ‘alaihi)

As for its legal ruling: is siwak obligatory? Imam Nawawi answers this clearly in Syarh Shahih Muslim: “Siwak is sunnah and is not obligatory under any circumstances, whether before prayer or in any other situation.” This means using siwak is a highly recommended act of worship but not sinful if skipped.

Nature’s Laboratory: What’s Inside a Siwak Twig?

This is where things get truly remarkable. Modern science has confirmed that siwak is no ordinary twig. It is a natural pharmaceutical laboratory packed with active compounds, each with a very specific function for oral health.

Fighting Bacteria and Preventing Cavities

Siwak contains alkaloids (salvadorine) that are bactericidal literally destroying the cell walls of harmful bacteria such as Streptococcus mutans, the leading cause of tooth decay. The compound isothiocyanate prevents bacteria from adhering to the tooth surface and forming biofilm (plaque). Meanwhile, trimethylamine suppresses acidity levels in the mouth, creating an environment hostile to bacterial growth.

A study cited in the Journal of Ayurveda and Integrative Medicine found that regular siwak use was more effective in reducing harmful bacterial colonies than using a regular toothbrush without antibacterial toothpaste because siwak slowly releases its active compounds each time its bristles come into contact with saliva.

On top of that, the natural fluoride in siwak triggers remineralization which is a process by which minerals re-enter weakened tooth enamel — making teeth harder and more resistant to acid.

Naturally Whitening Teeth

The yellowing of teeth is often caused by the buildup of stains from food and drinks. This is where silica in siwak plays its role: acting as a gentle micro-abrasive that lifts stains without damaging the tooth’s protective layer. The result is naturally whiter teeth not through chemical bleaching, but through thorough and deep cleaning.

Furthermore, the resin (essential oils) released during use forms a thin protective coating over tooth enamel, shielding teeth from the acid in food for several hours after each use. Its distinctive natural scent also means fresher breath that lasts noticeably longer compared to a regular toothbrush.

Strengthening the Gums

Vitamin C in siwak acts as an antioxidant that accelerates the healing of inflamed or injured gum tissue (gingivitis). Tannins (tannic acid) are astringent — they tighten gum tissue and close its pores so gums are less prone to bleeding under pressure. Calcium and chloride are also present to maintain mineral balance in saliva, keeping teeth strong from the inside out.

Stimulating Better Saliva Production

The act of chewing the tip of the siwak stimulates the parotid glands to produce more saliva. This is no small detail — saliva contains lysozyme (an enzyme that breaks down bacterial cell walls) and bicarbonate (a natural pH buffer) that together create a chemical defense system inside the mouth throughout the day.

How to Use Siwak Correctly

For those who want to give it a try, here is a step-by-step guide:

Preparation: Peel the bark of the siwak twig about 1 cm from the tip.

Making the Bristles: Chew the peeled end until it forms fine bristles resembling a brush head. If it feels too hard, soak it briefly in clean water to soften.

Cleaning the Teeth: Scrub all surfaces of the teeth and gums using vertical or circular motions, just as you would with a regular toothbrush.

Cleaning the Tongue: Use the siwak to sweep the surface of the tongue, removing bacteria and food residue.

Maintenance: After use, trim the used bristle tips and store the siwak in a clean, dry place.

Siwak in the Modern Era: A Tradition That Keeps Evolving

Despite the abundance of high-tech toothbrushes on the market today, siwak remains relevant precisely because it contains active compounds that conventional toothbrushes simply cannot replicate. This awareness has driven a wave of modern innovations built around siwak.

First, siwak-based toothbrushes. In Saudi Arabia and several other Middle Eastern countries, toothbrushes are now available with plastic or metal handles just like regular ones, but with bristle heads made from processed siwak fiber. The concept preserves the natural properties of siwak while delivering the convenience of a modern toothbrush. The brush head is even refillable, keeping it hygienic without the need to peel a new twig every time.

Second, toothpaste with siwak extract. This innovation allows anyone to enjoy the benefits of siwak’s active compounds while still using a conventional toothbrush. Siwak-based toothpaste is now widely available in many countries, including Indonesia, serving as a bridge between ancient tradition and modern convenience.

Both innovations share one common goal: to ensure that this Prophetic legacy does not only live on in the pages of hadith collections, but continues to be present and beneficial in everyday life.

Closing: A Tradition That Has Never Gone Stale

Siwak is proof that ancient wisdom does not always lag behind the march of progress — sometimes, it is modern science that eventually catches up and confirms it. From its antibacterial compounds and natural fluoride to its enamel remineralization mechanism, siwak offers a comprehensive oral care package, all packed into one simple twig.

More than just a cleaning tool, using siwak is a practice that connects us to the Sunnah of the Prophet — a small daily habit that, when done with sincere intention, can carry the weight of worship while keeping us healthy for years to come.

Written by: Alhayya Maritza

REFERENCES

Abdullah, N., Abubakar, S., & Muchtar, A.
